What is abnormal bleeding?

Abnormal bleeding describes any uterine bleeding that occurs outside of your normal menstrual cycle. It also includes any heavy bleeding associated with your periods or if your periods last longer than seven days.

There are a number of underlying issues that cause abnormal bleeding, such as:

  • Polyps
  • Cancer
  • Fibroids
  • Pregnancy
  • Thyroid disorder

Endometriosis is another common cause of abnormal bleeding. This condition develops when the uterine lining grows outside of your uterus. Even out of place, the lining continues to function like it does inside the uterus, thickening and breaking down each month. 

However, the displaced tissue has nowhere to escape the body and leads to chronic pain, abnormal bleeding, and, possibly, infertility.

Both teenagers and women heading into their menopausal years are also likely to develop abnormal bleeding due to a hormone imbalance.

How is abnormal bleeding diagnosed?

To determine the root cause of your abnormal bleeding, your RPW Obstetrics and Gynecology provider discusses your personal and family medical history. 

They may do a pelvic exam to check your uterus and surrounding structures for abnormalities and run blood tests to check hormone levels.

The RPW Obstetrics and Gynecology staff offer on-site diagnostic imaging tests, like ultrasounds, that can show fibroids or polyps in the uterus that may cause abnormal bleeding. If the ultrasound isn’t enough to identify the issue, you may need a hysteroscopy.

During a hysteroscopy, your RPW Obstetrics and Gynecology provider inserts a thin tube with an attached camera and guides it into your uterus. 

The camera sends back real-time pictures of your inner structures to an external monitor that your doctor watches throughout the test. A hysteroscopy can identify abnormal growths like fibroids, or conditions like endometriosis.

In some cases, your RPW Obstetrics and Gynecology provider may do a biopsy, a procedure to remove a small sample of uterine tissue for analysis in a medical lab.

How is abnormal bleeding treated?

Depending on the cause of your abnormal bleeding, your RPW Obstetrics and Gynecology provider may recommend one or more treatments to stop bleeding. This may include birth control pills or an intrauterine device (IUD) to rebalance hormones and thin out your uterine lining.

You may also need a minimally invasive procedure known as dilation and curettage (D&C) to scrape away some of the excess uterine lining to treat heavy or abnormal bleeding. 

The doctors also perform endometrial ablation to destroy a layer of your uterine lining and minimally invasive surgeries to address fibroids or other endometrial growths.
